*NOTE: Registration forms filled out with mobile devices will produce an error message upon submitting the form. Please use a computer for these steps to assure your registration forms are completed and filed correctly. We recommended using Google Chrome to complete these steps in the registration.
- Health Portal (CLICK HERE)
Freshmen/Transfer Students Need:
-Physical Exam (within 6 months of arrival on campus)
-Immunization record
-ADD/ADHD Medical Exemption Form
*If taking ADD/ADHD medication, also need ADD/ADHD Medical Exemption Policy and Physician Form
-Assumption of Risk Form
-Consent to Treat Form
-HIPPA/FERPA Form
-Insurance Information Form
-Student-Athlete Medical History Form
-Sickle Cell Form
*Also need one of the following: newborn sickle cell results, a new sickle cell test with results, or sickle cell waiver
-Complete the "Insurance" tab
-Upload a copy of the front and back of your insurance card
-Baseline concussion testing (done once on campus)Returners Need:
-Physical Exam (within 12 months/1 year)
-ADD/ADHD Medical Exemption Form
*If taking ADD/ADHD medication, also need ADD/ADHD Medical Exemption Policy and Physician Form
-Assumption of Risk Form
-Consent to Treat Form
-HIPPA/FERPA Form
-Insurance Information Form
-Student-Athlete Medical History Form
-Update the "Insurance" tab
-Upload an updated copy of the front and back of your insurance card
